Fees for patients without insurance are determined from a sliding fee scale based upon income and family size.

Our Sliding Fee Scale Program offers reduced healthcare payments for eligible individuals and families. It is based on the Federal Poverty Level guidelines. This program may reduce an individual's bill 20%-80%.

Wood River Health provides a full array of primary and preventive services for infants to adults. Some of these services include pediatrics, prenatal care, women’s health care and many other specialty services.

You can contact your insurance carrier and request a change in your assigned primary-care physician and contact your current physician to have your records transferred.

All services are provided regardless of the patient’s ability to pay. Qualified patients will be charged according to the agency’s sliding fee schedule. Staff is available to help patients access health insurance and answer any questions about payments.

We are committed to clearly and thoroughly communicating with all patients about the care we provide. Please let your provider know in advance about your language or hearing needs.

Interpreter services for non-English speaking and hearing impaired patients is available. Interpreter services for non-English speaking patients are available in each of our exam rooms through phone.

Sign language services are available for hearing impaired patients, we ask that you please give us 48 hours of notice if you require this service.

Patients must bring a valid form of ID, proof of address, list of medications and insurance information. If you plan to apply for our Sliding Fee Scale Program, please bring proof of income.

Yes. Children younger than 18 need parental consent for visits and a parent or guardian needs to be present for primary-care visits or annual physical examination visits.

To apply for this program, please bring provide proof of income including your last three paycheck stubs, copies of entitlement checks and your W2 Form.

If you have questions about insurance eligibility, our Outreach and Enrollment Specialists are here to help get them answered. They also help patients navigate the enrollment process through HealthSource RI, Rhode Island’s Health Insurance Exchange. These services are free and confidential.

Wood River Health uses an Electronic Medical Record (EMR). This enables us to streamline the management of patient information and allows us to make quality improvement efforts in primary care.

An EMR provides a seamless interface for the integration of comprehensive medical, behavioral health, dental and pharmaceutical information. It has greatly enhanced Wood River Health’s outcome measurement capacity and has been a vital link in Wood River Health’s clinical quality program, including risk reduction, practice evaluation, patient education and provider education.

The EMR is in alignment with The Rhode Island Health Information Exchange Act of 2008 and with statewide efforts in health information technology including optional participation in CurrentCare, the statewide health information data exchange system. CurrentCare allows healthcare providers who use electronic records to share data and provide coordinated care of patients should they go to a lab, hospital or emergency care center. This avoids errors and duplication of services while improving quality of the care.

  • Tell your team if you are having trouble sticking with your care plan.
  • Speak up if your care plan is not working. Tell your team what is not working so together you can make changes if needed.
